IN-HOME EVALUATION SERVICES

The Adult Evaluation and Review Services (AERS), a state program sponsored by the Howard County Health Department, provides screening, consultation, free multi-disciplinary assessment and access to public services for persons with disabilities and frail adults needing support to live in the community. This is a critical step determine eligibility for in-home, assisted living and nursing home care.

  • Through the assessment and recommendations of a Registered Nurse or Licensed Clinical Social Worker, an AERS evaluation can put you in touch with valuable programs and services for the best and most appropriate care. This service is free to Howard County residents and is offered in the resident's own home.
